Locating the femoral nerve on overweight patient
I am struggling to locate the femoral nerve on my overweight patient. I have tried to find the saphenous nerve but I don’t get a contraction.
Any suggestions?
Hi Elize! It can be quite the challenge to locate it on an overweight patient.
Palpate for femoral artery and the nerve is just lateral to it. Use increased pressure and set the Stimpod’s pulse width to 0,2ms, stimulations per second to 2Hz and adjust the current as high as the patient can endure.
Remember that the saphenous nerve is sensory only. As long as the patient feels it radiating towards the knee, you know you are treating the nerve. There will be no fasciculation.

For saphenous nerve treatment, you can also go lower down the leg, just medial to the knee. Have a look at this treatment protocol which shows the treatment area.
I managed to get it today. I think it actually might have been some recovery of the nerve from the last 2 sessions as nothing changed about my application👏

Just a comment – if you know the anatomy and yet you don’t see any activity in the nerve – just keep going – it will come back!
